Install and connect
- Install: Add MasterForms from the monday.com marketplace.
- Add the view: On your board, click + next to the board's views, choose Apps, then MasterForms.
- Connect once: The first time it opens, press Connect to monday and approve. MasterForms needs this to read the board's columns and create items from responses. If nothing happens, use Open it in a separate window.
- Start building: You land on the Form builder. Tick your first column, press Start from a template, or press Or describe the form and let AI build it.
Note: Connecting is done once for the whole monday.com account. After that, everyone on the account goes straight to the builder.
- One form per board view. To have two forms on one board, add the MasterForms view twice. Each view keeps its own form.
- New items are created on the board the view belongs to, in the group you choose in Form settings → Responses.
- Team members, not guests, can import forms, hand out item links, put links on the board, approve responses in monday, and see responses and fill in forms in the item card.
Where things are
| In the app's menu | What it's for |
|---|---|
| Form builder | Choose the questions, arrange them into pages and set each one up. Switch to Logic to see every rule on one map. Share is at the top. |
| Form settings | How the form works and how it looks, in six tabs: General, Responses, Editing, Approvals, Appearance and Languages. See the table below. |
| PDF template | Print each response onto your own PDF, attach it to the item and save it to Google Drive. |
| Emails | Send respondents a copy of their answers, and tell your team about new responses. |
| Dashboard | A summary in words, headline figures against the period before, responses over time, every answer (click one to see only those responses), visitors, and when and where responses come from. Open any chart full screen, choose the chart colours, and ask AI about the answers. |
| Responses | Every response in a list, with search and CSV export. |
| Google Drive | At the bottom of the menu. Shows whether Drive is connected for the PDFs. |
Tip: Drag the menu's right edge to make it wider or narrower; double-click it to reset. Drag it narrow enough and it shows icons only.
Form settings has its tabs across the top and shows one tab at a time:
| Form settings tab | What's on it |
|---|---|
| General | The title and description, Availability (when the form is open, the maximum, finish later and one response per person) and Access (who fills it in, and a password). |
| Responses | Where responses go, Sort each response with AI, After submitting and Pre-filled links. |
| Editing | Editing after submission and Updating existing items. |
| Approvals | Approvals: up to three steps before anyone acts on a response. |
| Appearance | The look: ten themes, your logo, organisation name and cover image, colours, fonts and shapes, and one question at a time, with a live preview. |
| Languages | Offer the form in up to 10 more languages, translated by AI. |
Inside an item card, the MasterForms view shows the response that made the item and the forms your team fills in on it. See In the item card.
