Getting started
What does MasterForms do?
It turns a monday.com board into a form you can share with anyone. Each column you tick becomes a question, and each response becomes a new item on the board with the answers already in the right columns. People filling in the form don’t need a monday.com account.
How do I get MasterForms?
MasterForms is coming soon to the monday.com marketplace. Contact us with any questions in the meantime.
How do I set it up?
On your board, click + next to the board’s views, choose Apps, then MasterForms. Press Connect to monday once, tick the columns you want to ask about or start from a template, then press Publish and Share. See getting started.
Who on my team can use it?
Connecting is done once for the whole monday.com account. After that, everyone on the account goes straight to the builder. Importing forms, handing out item links, putting links on the board, approving in monday, and seeing responses and filling in forms in the item card are for team members, not guests.
Can I have more than one form on a board?
Yes. Each MasterForms board view is its own form. Add the view again for another form.
Do I have to use AI?
No. Every AI feature is optional: building, importing, translating, sorting and Ask AI. Nothing is sent to AI unless you use one of them.
Building the form
Which columns can be questions?
Name, Text, Long text, Email, Phone, Link, Number, Rating, Status, Dropdown, Date, Checkbox, File, People, Connect boards, Dependency and Subitems. Formula and Mirror columns show a value on the form, and an Update question posts its answer as an update on the new item. Country, Tags, Color, Timeline, Week, Hour, Location, World clock, Time tracking, Vote, Progress and columns monday.com fills in itself can’t be questions. See which columns can be questions.
Can I add a column the board doesn’t have yet?
Yes. Press + New column at the bottom of the column list, give it a name and a type, and it is added to the board and to the form. Select all adds every column at once.
How do pages and layout work?
Press Add page to split the form into pages, with a progress bar for respondents. Drop one question on another to put the two side by side, two to a row. Text, Image and Divider blocks go between questions and collect nothing. A ticked column is added to the last page, and pages can’t be renamed or reordered, so build them in the order people will fill them in.
Can I start from a template?
Yes. On an empty form, press Start from a template and choose one of eight: Service or maintenance request, Customer feedback (NPS), Job application, Event registration, Client onboarding, IT help request, Expense claim and Contact us. You review every question and where it’s saved before anything is built; columns the board doesn’t have are created when you confirm. See templates.
Can AI build the form for me?
Yes. Press Build with AI and describe the form, and AI picks the matching columns that are already on your board. To import a form you already have, upload a PDF (up to 10 pages), a Word file or up to 5 photos of a paper form, paste a public Google Form link, or paste the questions. You review every question and choose its column before anything is built. See build a form with AI.
Can people sign the form?
Yes. Under the form, Add to this page → Signature adds a box people sign with a mouse or a finger. The drawing is saved as an image in a File column on the item: one the form doesn’t use yet, or a new Signature column. You can also choose Signature as the type under + New column, or turn on Collect a signature on a File question. See signatures.
Does importing a form add columns to my board?
Only the new columns you confirm. They are added as soon as you confirm, and stay on the board even if you don’t keep the form. Importing replaces the questions already on the form.
What happens when I change a form people are using?
Nothing changes for them until you press Publish. Your changes save as a draft a couple of seconds after you stop. If someone is filling in the form when you publish, they are asked to reload when they send, and their answers are kept on their device.
Can I try the form before publishing?
Yes. Preview opens your draft in a new tab so you can try it like a respondent. Nothing sent from a preview reaches the board, and a preview link works for 30 minutes.
Can I build a survey?
Yes. A Rating column shows stars or hearts. A Number question can be a 1 to 5, 1 to 7, 1 to 10 or 0 to 10 scale, with words under the lowest and highest number. Status and Dropdown questions can show their choices as buttons. Turn on One question at a time for a screen per question with a progress bar. See surveys.
Can the form work out totals?
Yes. A Formula question shows its result as people type, for example a budget with tax, as long as every column the formula uses is also a question on the form. Formulas that use dates are worked out by the board once the answers are saved. A Mirror question shows the value from the item picked in a Connect boards question. See calculations.
Rules and logic
Can I show a question only to some respondents?
Yes. Under Show this question only when, add a condition on an earlier answer, such as Request type is Repair. Add more conditions and join them with AND (all must be true) or OR (any one is enough), up to 20 per question. Text, image and divider blocks can have rules too. See rules.
Which answers can a rule check?
Status, Dropdown, Checkbox, Number, Rating, Date, People, Connect boards, Dependency and every kind of text answer. For example: a number is at least 5, a date is within the next 3 days, a text answer contains a word, or a dropdown has at least 2 picked. Files, signatures, line items, formulas and mirrors can’t be used in a rule.
Can I skip a whole page?
Yes. From page 2 on, a page can have its own conditions. A skipped page is left out of the step count, Next goes straight past it, and every question on it is hidden. A page’s conditions can only use questions on earlier pages. See skip whole pages.
Can one question’s options depend on another answer?
Yes. Limit options based on another answer offers only some of a Status or Dropdown question’s options, depending on an earlier Status, Dropdown or Checkbox answer. When only one option is left, it is picked for the respondent with a short note. See limit options.
Can a dropdown list values from another board?
Yes. Options from another board fills the list from a column on another board. You can keep only the rows that match another answer on the form, such as the sites of the customer that was chosen, or only rows with a fixed value. See options from another board.
Can I show something only to people with a certain link?
Yes. A Link parameter condition shows a question or block only when the form link includes a value, such as &ref=partner. Anyone can change a link, so don’t use this to hide anything private. An Answered count condition shows something once enough questions are answered.
How do I check my rules work?
Switch from Form to Logic above the form. You see every page and question in order, with arrows from each answer to what it changes, and a list of rules that can’t work as intended, each with what to change. MasterForms also checks the form when you publish. See the Logic view.
What happens to answers to hidden questions?
Hidden questions are never required, and their answers aren’t saved, even if they were filled in before being hidden. The rules are checked again when the response arrives, so the board only gets answers to questions that were shown.
Responses and editing
Where do the answers go?
Each response creates an item on the board the form was built on, in the group you choose under Form settings, with every answer in its column. Lines added to a line items question become subitems of that item.
How do line items work?
Tick the board’s Subitems column, then switch to Subitem form and tick the subitem columns to ask about. On the form, people add as many lines as they need, up to 20, and each line becomes a subitem. You choose whether people editing a response may add, change or remove lines. See line items.
Can someone change their answers after sending?
Yes, when Let respondents edit their submission is on. After sending, they get a private link that reopens their answers, and saving updates the same item instead of creating a new one. Maximum edits per submission stops the link after that many edits. See edit links.
Can I stop some answers from being changed later?
Yes. Open the question and turn off Can be changed later. Whenever a response is changed, through an edit link, a private item link, finding the item or the item card, that question shows its first answer, greyed out and marked "Can’t be changed". If it was left empty, it can still be answered once. Signatures and files can’t be replaced once sent. Your team can still change the column on the board.
Can people finish a long form later?
Yes, with Let people save and finish later. The Save and finish later button gives a private link that brings the answers back on any device for 30 days. Files and signatures aren’t saved, so people add them when they come back. Separately, answers are kept in the browser as people type: if someone closes the tab by mistake and comes back on the same device, they are asked whether to pick up where they left off.
Can I stop people responding twice?
Yes. Turn on One response per person and choose the question that recognises them, such as their email, then make that question required. Under When someone responds again, choose Refuse it, Add it to their first response (the new answers are posted on their first item as an update, nothing is overwritten) or Replace their first answers. MasterForms keeps only a scrambled fingerprint of the answer, never the answer itself.
Does a repeat response count as a new response?
No. A repeat added to the first response, or replacing it, makes no new item. It doesn’t count against Maximum submissions, and emails, AI sorting, approvals and the When a form is submitted trigger don’t run for it. With Replace their first answers, anyone who types the same answer, such as someone else’s email, can change that person’s answers, so use it on forms for your team or with a password.
Can I limit when the form is open?
Yes. Turn off Accepting responses to pause it, set Opens on and Closes on dates, or set Maximum submissions. Outside those limits the link shows “This form is closed”. See availability.
Can I protect the form with a password?
Yes. Under Access, turn on Require a password and set one of at least 4 characters. Visitors enter it before the form is shown.
Can I fill in answers from the link?
Yes. Add &p_ plus the column’s ID, an equals sign and the value to the form link, for example &p_email=dana%40acme.com. Respondents can still change a pre-filled answer, and Form settings shows an example for your form. See pre-filled links.
What do people see after sending?
Your own heading and message. You can also send them on to another page, such as a thank-you page, and offer a Submit another response button. With editing on, the confirmation shows their private edit link.
Updating existing items
Can people update an item that’s already on the board?
Yes, in three ways, turned on under Form settings → Editing → Updating existing items. Let people find their answers: they enter two answers, such as their email and order number, and the matching item opens filled in. Send a private link for each item: each item gets its own link. Open this form inside the item card: your team changes items from inside monday.com. See update existing items.
How safe are the item links?
Each item link opens that item without anything else to know, so send it only to the right person. Links can stop working a number of days (1 to 365) after they were last sent, or after one save, and you can turn off any single link. For Let people find their answers, choose two answers that are hard to guess together: a miss never says which answer was wrong, and too many tries are blocked for a few minutes.
Can my team use the form inside monday.com?
Yes. Turn on Open this form inside the item card and publish, then add MasterForms as a view in any item card and choose Team forms. The form opens filled in with that item. It is for members of the account, not guests or viewers, and it keeps working while the form is paused for everyone else. For a form only your team fills in, see forms for your team.
Can I send a link to everyone on a board and see who has answered?
Yes. With Send a private link for each item on, Collect from everyone on the board puts each item’s private link in a Link column and keeps a Form status column up to date: Not sent, Sent, Opened, Done. Press Put links on N items for the whole board or one group, and send the links and reminders with monday automations on the Form status column. See update existing items.
Does updating an item change other columns?
No. All three ways use the form’s rules and limits and save only the questions on the form. Other columns on the item are left alone.
Team forms and the item card
Can my team fill in a form on an item, without a public link?
Yes. In Form settings → General → Access, set Who fills it in to “Your team, in the item card” and publish. The form then opens only in the item card: open an item, choose MasterForms, then Team forms. Saving changes that item. It has no public link, so its link, edit links, private item links, finding your item and save and finish later all refuse it, and the password setting is hidden. See forms for your team.
What is a team form good for?
Anything your team fills in on an item rather than a customer: a visit report, an inspection checklist, a handover. For example, a technician opens the job’s item in monday.com on a laptop or tablet, fills in a checklist, adds photos and has the customer sign. The answers go into the item’s columns, and with a PDF template the signed report is attached to the item. See use cases.
Can I see what someone sent from inside monday.com?
Yes. Add MasterForms as a view in the item card. When a form made the item, its Response tab shows the form’s name, every answer in the form’s order (files as links, the signature as an image, line items as a table), when it was sent and edited, which link it came from and how long it took, with Open the PDF, Copy their edit link and any approval. Answers are read from the item as it is now. See in the item card.
Who sees the item card view?
Members of the account, not guests or viewers. On an item no form made, the Response tab says No form made this item, and Team forms lists the forms your team can fill in there.
Approvals
Can responses be approved before anyone acts on them?
Yes. In Form settings → Approvals, turn on Send each response for approval, choose an Approval column and set up to three steps, such as Manager, then Finance. Each new response waits for each step in turn: approving moves it on, rejecting stops it. See approvals.
Who can approve?
For each step, choose Chosen people (up to 10 people in your account, any one of whom can decide), The person in a People column on the item (such as its Manager), or Someone outside your account, who gets a private link.
How do people approve?
People in your account open the item and choose MasterForms in the item card: the approval is at the top of the Response tab, above the answers. They press Approve or Reject. Someone outside your account opens their private link, which shows the answers, enters their name and decides. A comment is needed to reject. Every decision is posted on the item as an update.
How do approvers know a response is waiting?
The Approval column shows where each response is, such as “Waiting: Finance”, then “Approved” or “Rejected”. Use it in monday.com automations: for example, notify the finance team when it changes to Waiting: Finance, or email the approval link when it changes to Waiting: Client.
Are responses already on the board sent for approval?
No. Only responses sent after approvals are published go for approval. Edits, and repeat responses added to a first response, aren’t sent again.
PDFs, automations and AI sorting
Can each response produce a PDF?
Yes. Upload your own PDF on the PDF template tab, such as a work order or a contract, and drag each question to where its answer should be printed. It doesn’t need fillable fields. Each response fills the PDF and attaches it to the file column you choose, named after the item, and it is made again when a response is edited. See the PDF template.
Which alphabets print on the PDF?
Latin, Greek, Cyrillic, Hebrew and Arabic, including right-to-left text.
Are signatures printed on the PDF?
Yes. Drag the signature question onto your PDF template, where the person should sign, and set its width. It prints as the respondent drew it. If someone edits their response without signing again, the PDF uses the signature already on the item. File uploads, updates and line items can’t be placed on the PDF.
Can the PDF be saved to Google Drive?
Yes. Under File it in Google Drive, press Connect Google Drive and paste the folder link or ID. Drive is connected once for the whole account.
Can a response start a monday.com automation?
Yes. MasterForms adds a When a form is submitted trigger, and two actions: one puts the respondent’s edit link in a Link column, the other puts the item’s update link in a Link column. Find them in your board’s Integrate menu by searching for MasterForms. See automations.
Does editing a response run AI sorting and automations again?
No. AI sorting, approvals and the When a form is submitted trigger run for new responses only. The PDF is made again on every edit, so it always matches the item.
What does AI sorting do?
It reads each new response and fills the columns you choose: Category, Urgency and Sentiment (Status columns) and a one-sentence Summary (a Text or Long text column). It only picks labels your columns already have, so monday.com automations can route the response. See AI sorting.
Languages and branding
Can the form be in other languages?
Yes, in up to 10 more languages. AI translates every text on the form, including buttons and messages, and you can review and change any of it. People see the form in their browser’s language or choose one at the top, and answers reach the board in your original words. Add &lang= and a code, such as &lang=de, to link straight to a language. See languages.
Can I match the form to my brand?
Yes. In Form settings, on the Appearance tab, start from one of ten themes (Classic, Clean, Warm, Calm, Bold, Dark, Editorial, Blush, Midnight and Forest), all in the calm style of Jira’s forms, then change anything: your logo and organisation name on a band across the top, a cover image, over 100 fonts plus a heading font, three text sizes, colours, corners, answer fields and button shape. A preview beside the options shows each change as you make it. Status choices can show your board’s own label colours. Right to left is available for Hebrew, Arabic, Persian and Urdu, and AI can pick the colours and a font from your logo. See appearance.
Will a theme change forms I already have?
No. A form only changes when you pick a theme for it. Forms without a theme look exactly as they did.
Is there a “Powered by” footer on the form?
No. The form shows only your logo, colours and text.
Do the fonts load from Google?
No. Fonts are served by MasterForms, so respondents’ browsers don’t contact Google.
Dashboard, data and privacy
What does the Dashboard show?
A one-sentence summary of what came in, then responses, the share of visitors who send the form, the Net Promoter Score (or average rating) and how long people take, each compared with the period before. Responses per week (per day over 7 or 14 days) show the period before as a dashed line. Every question gets a card with its answers; click one and the whole Dashboard shows only those responses. It also shows how many people reached each page, where people stop, the busiest hours of the week, and which links responses came from. Click a chart or a headline figure to open it full screen, with more ways to draw it and a table of every value, and choose the chart colours: Default, Vivid, Cool or Soft. Compare two questions, and ask AI about the answers. See the Dashboard.
Can I ask questions about the responses?
Yes. Ask in your own words, like “What priority are most requests?”. Counts and averages are worked out exactly, and AI reads written answers only when you ask about themes. Your account can ask 100 questions a day.
Can I export the responses?
Yes. Export CSV downloads every response with the time it was sent, when it was last edited, how many edits, one column per question and the item ID. It opens correctly in Excel, accents and all.
Where are the answers kept?
In your monday.com board: each response is an item. MasterForms keeps your form’s setup, a record of each response (when it was sent and edited, the link tag it came with and how long it took), each response’s approval decisions, saved finish-later answers for 30 days, and visit counts for drop-off.
What is sent to AI?
Only what the feature you use needs, sent to AI models through OpenRouter. AI sorting never sends names, email addresses, phone numbers, people, files or signatures, and Ask AI never sees emails, phone numbers, links, files or signatures.
What are the limits?
100 questions and blocks per form, 2 questions side by side, 20 conditions per question or page, 20 lines per response, 10 files per response at 10 MB each, 10 extra languages, and 3 approval steps with up to 10 chosen people each. AI import, translation and sorting can be used 500 times a day per account. See limits.
